Paperwork Guidelines – LABEL IS THE LAW #

Paperwork is highly regulated. Graphs should be done with a straight edge. All paperwork should reveal exactly what was done at the property. Below is the required information needed on paperwork for a job in Louisiana.
Louisiana Paperwork Law and Guidelines Below #
Records for applications of pesticides for wood-destroying insects shall contain the following information:
- Place of business name, address, and number.
- Primary licensee name, address, and department I.D. number.
- Customer name and address.
- Location of application; product/brand name; EPA registration number; application date and time.
- Restricted/general use pesticide.
- Target pest: type of application (pre-treat, post, spot, etc.).
- Size of area treated (square feet or linear feet).
- Mixture concentration.
- Total amount of product mixture applied.
- Applicator and department I.D. number.
- Contract.
Record Keeping for Applications of Pesticides in the General #
Pest and commercial vertebrate phases shall contain:
- Place of business name, address, and number.
- Primary licensee name, address, and certification department I.D. number.
- Customer name and address.
- Location of application; product/brand name; EPA registration number.
- Restricted/general use pesticide.
- Application date and time; pest treated/type of application.
- Mixture concentration (percent).
- Applicator and department I.D. number.

Pest Control #
Complete as instructed. Be sure all products are properly labeled with the correct gallons, %, target area, and target pest.
Termite Related #
Take a picture of the 4 corners of the home. Stand approximately 15 feet back from each corner. Label each picture (front left, front right, back left, back right). Additionally, take pictures of conducive conditions and properly label them in the “conditions” section with their location.
Before a termite renewal service, check the previous treatment and renewal history. If a conducive condition has been written up more than once, notify the homeowner and educate them.
Paper #
Take a picture of any graph or hand-written proposal. Be sure it is legible. Label it properly on the mobile.
TAT & CATs: Be sure any TAT or CAT treatments are properly labeled with the target pest and not “termite.”
Texas Paperwork Specific Notes: #
- Before any full termite treatment, be sure to get the termite disclosure form signed. It is a requirement. Front and back of the form.
- There is a separate disclosure for pre-construction and post-construction.
- For pre-construction borate or soil pre-treats: a 1-year perimeter treatment is not required by our regulatory agency. We must follow up once the structure is completed and try to get the new homeowner info to transfer the termite policy to them from the builder’s name. We also need to quote our renewal info, pest specials, and upgrades to Sentricon. If they renew with a liquid, we need to perform the perimeter trench and treat at the same time.
- Commercial pre-treat call-ins: We must call in commercial pre-treatments a minimum of 4 hours before the application should start. Do not start the application before the time that is called in.
Mississippi Paperwork Specific Notes: #
- Pre-treats: utilize the Mississippi pre-treat calculation form. Must fill out the front and back completely.
- Post-construction termite treatments: utilize the Mississippi termite treatment calculation form.
- Be sure the graphs are precise.
- Renewal: every termite renewal in MS, we must attempt and offer to inspect the inside of the structure. Be very attentive to these inspections and documents. Mark this attempt or inside inspection on your paperwork.
